چکیده

SINUS bradyeardia has been associated with a number of entities, both normal and pathologic; however, it is usually of a transient nature. It has been found in 30 per cent of healthy young males,' the well-trained athlete,2 and has been associated with pregnancy, particularly the post partum period.3 Its incidence markedly exceeds what might be expected in starvation, myxedema, jaundice, increased intracranial pressure, beriberi, labyrinthitis, convalescence from certain infectious diseases, and bilateral thoracic sympathectomy.4-8 In a series of 6,786 medical hospital admissions Kirk and Kuorning' found sinus bradyeardia in 515 patients, or 7.6 per cent; syncope due to the sinus bradyeardia occurred in only 1 patient. Sinus bradyeardia due to arteriosclerotic heart disease is rare. As can be seen from examination of the above list of entities, there are diverse causes of sinus bradyeardia. These causes may be classified as neurogenic or non-neurogenic. By blocking of vagal activity with atropine increased vagal activity has been demonstrated in young men,7 starvation,4 and increased intracranial pressure.8 Short9 describes 4 patients with alternating sinus bradyeardia associated with a variety of tachyeardias. Sinus standstill was present in 2 patients and a wandering pacemaker in 3 patients. Atropine, effort, or emotion was found to accelerate the rate considerably, usually to a normal sinus rhythm, in all these patients. Asystole with normal sinus rhythm and nodal rhythm have been described respectively by Laslett'0 and Wedd and Wilson."1 These

Cardiac arrhythmias during or after epileptic seizures

Seizure-related cardiac arrhythmias are frequently reported and have been implicated as potential pathomechanisms of Sudden Unexpected Death in Epilepsy (SUDEP). We attempted to identify clinical profiles associated with various (post)ictal cardiac arrhythmias. We conducted a systematic search from the first date available to July 2013 on the combination of two terms: 'cardiac arrhythmias' and ...

Sotalol-induced torsade de pointes: management with magnesium infusion.

A 69 year old woman was treated with sotalol (320 mg daily) for intermittent atrial fibrillation. Sotalol was initially well tolerated and reversion to sinus rhythm with sinus bradycardia occurred 4 weeks after initiation of therapy. Shortly thereafter, the patient developed recurrent syncope due to torsade de pointes. This was treated successfully with intravenous magnesium infusion and withdr...

Autonomic and electrocardiographic changes in cardioinhibitory syncope.

AIMS Cardioinhibitory syncope (CS) is a neurally mediated response causing bradycardia or asystole. This study reports on changes in blood pressure, heart rate variability (HRV), and ECG patterns before and after syncope with asystole. METHODS AND RESULTS Thirty-five patients with CS and a matched control group were submitted to 60 degrees head-up tilt for 20 min with the addition of nitrogly...

Mechanism of syncope in patients with heart disease and negative electrophysiologic test.

BACKGROUND In patients with syncope and structural heart disease, syncope is suspected to be attributable to a primary cardiac arrhythmia, but little is known of its mechanism when electrophysiologic study is unremarkable. METHODS AND RESULTS We applied an implantable loop recorder in 35 patients with overt heart disease at risk of ventricular arrhythmia, because these were patients with prev...
